Communications support request form

Instructions

Use this form to describe any communications activity that is likely to require more than a few hours of Sea Grant Communications staff time.

Such needs may include publications or Web pages, writing or editing assistance, audiovisual products and services, computer-related products or projects, display and exhibit materials, and conference and event support. (You generally do not need to submit a form for a news release or other minor requests.)

To the best of our ability, communications staff will respond to your request within five working days. At that time we will tell you:

We encourage planning of communications activities, beginning early in the life of the project and including a careful evaluation of the need, the audience, the message, and the medium. You need not complete this form for such a consultation -- just contact Rick Cooper (for print projects), Pat Kight(for Web projects), Patricia Andersson(for graphic arts projects) or Joe Cone (for video projects) and set up an appointment.

1. Provide a brief descriptive name for your requested activity: e.g., Tsunami publication, Tuna conference, Fish Festival Web site

2. Describe the purpose of the communications product or assistance you would like from us. How will you be involved? Who else will be involved?

3. Describe the audience (who? where? how many of them?) for this communication activity and how you know they need it. Be specific.

4. Describe the response you expect from this audience. What do you expect to happen as a result of this activity, and why?

5. How will you/we evaluate whether you have achieved your purposes?

4. What priority do you place on this activity, relative to other communications activities you plan during the next year?

6. Details of the timing and funding

When do you want our work to begin?

Is there a firm deadline for completion of work?

What funds, if any, are available from you ior your partners for this work?

Who are your partners, and what roles will they play in this activity?

8. Who will be responsible for providing initial content? For Web pages, who will be responsible for ongoing content updates?

9. Are there other things that we should know in reviewing this project?
